Google’s Health Guardian features are no longer just a launch-event demo. Blood pressure trends, insulin resistance trends and sleep breathing quality began reaching Pixel Watch owners this week, per 9to5Google. All three arrive inside the Google Health app, and all three are free on the Pixel Watch 3, 4 and 5.

Health Guardian is Google’s umbrella name for this set of health and safety tools on Pixel Watch. Google previewed the suite at the Pixel Watch 5 launch on August 12, 2026. We covered the announcement and its caveats back in August, when all of it was still a promise. This week is the part that changes something on a watch you already own.
What Pixel Watch Health Guardian features actually do
Blood pressure trends does not take a cuff-style reading. The watch passively follows your pulse and motion patterns over the course of a month. From that, it estimates whether your blood pressure has drifted higher or lower. Google says the model behind it is called WavesFM, trained on billions of minutes of heart and movement sensor data.
Six in 10 people don’t realize when their blood pressure has shifted higher.
That line is how Google’s own announcement makes the case for the feature. Insulin resistance trends runs on a separate large sensor model. Google says it was pretrained on more than a trillion minutes of sensor data from five million people who opted in. Its accuracy was checked against standard clinical blood tests in a study published in Nature.
Sleep breathing quality is the third feature going out in the same wave.
The safety feature most readers won’t get
Breathing Emergency Detection is the fourth and most safety-critical piece of Health Guardian. It is limited to Europe, and per 9to5Google it works only on the Pixel Watch 4 and 5. Google’s own page describes eligibility more loosely, without naming which watches qualify.
Google’s footnote states the feature has not been cleared or evaluated by the FDA, and that it carries a CE mark for the EU. In practice, that means US owners do not get it at all. Pixel Watch 3 owners in Europe are also left out, on a watch Google is otherwise happy to add trend features to.
Fitbit Air owners will have to pay
The same three trend features reach Fitbit Air later this year. There, they sit behind a Google Health Premium subscription. So the identical health data is free on one Google wearable and a paid add-on on another. That is a two-tier split inside Google’s own hardware line, not a competitor gap.
Rollout timeline and one caveat
The three trend features started rolling out this week on Pixel Watch 3, 4 and 5. The first blood pressure and insulin resistance trend reports are expected in early October. Google’s own disclaimers are clear that these are wellness estimates. They are not diagnostic tools. They do not replace a proper cuff reading, a blood test or a doctor.
